October 12, 2026 (Houston Style Magazine) — In a city celebrated for medical innovation, world-class hospitals and Texas-sized compassion, Houston’s health leaders are proving that the most powerful prescription for a healthier community might be something surprisingly simple: working together.
On October 6, 2026, six influential organizations gathered at Healthconnect Texas headquarters to launch the Harris Collaborative, an ambitious partnership designed to transform healthcare delivery for Harris County’s most vulnerable residents.
The founding partners include the Houston Health Department, Community Health Choice, Harris County Public Health, The Harris Center for Mental Health and IDD, Harris Health and Healthconnect Texas.
For Houston Style Magazine, which has championed community health education, access and equity alongside Houston’s healthcare institutions for decades, this collaboration represents another promising chapter in the ongoing effort to make quality healthcare accessible to every neighborhood.
Six Organizations. One Healthier Houston. The Harris Collaborative tackles a persistent challenge: patients navigating disconnected healthcare, behavioral health and social-service systems.
Its solution? Connect the organizations, share information securely and coordinate interventions before preventable medical emergencies occur.
Healthconnect Texas, the statewide health information exchange serving more than 28 million Texans, supplies technology that helps participating organizations better understand shared populations and identify service gaps.
As Junaid Husain, CEO of Healthconnect Texas, explained: “The Harris Collaborative is about changing what happens to people before they reach a crisis.”
That philosophy reflects a powerful shift from responding to emergencies toward preventing them.
Three Priorities. Countless Lives. The Collaborative is concentrating its initial efforts on three populations:
Chronic disease patients: Individuals facing hypertension, heart disease and related risks, particularly in Northeast Houston and communities served by Lyndon B. Johnson Hospital.
Residents with complex healthcare needs: Individuals managing both physical and behavioral health conditions who face elevated risks of avoidable hospitalizations.
Justice-involved residents: People returning from the Harris County Jail who require coordinated medical, behavioral health and community support.
Harris Health’s initial Integrated Care Cohort specifically emphasizes improving cardiovascular outcomes, blood pressure management and preventive care in Northeast Houston.
Leadership That Puts People First Among the distinguished leaders celebrating the launch was Dr. Esmaeil Porsa, president and CEO of Harris Health, whose organization is helping facilitate the Collaborative’s mission.
He joined Keena Pace of The Harris Center, Leah Barton of Harris County Public Health, Junaid Husain of Healthconnect Texas, Lisa Wright of Community Health Choice and Dr. Theresa Tran of the Houston Health Department.
Together, their organizations represent an extraordinary collection of clinical expertise, public-health leadership, behavioral-health resources and technological innovation.
A Texas Model With National Potential The goal extends beyond Houston. Organizers envision developing a coordinated-care model that other Texas communities—and potentially communities nationwide—can adapt.
For families confronting chronic illness, limited resources or complicated healthcare systems, success could mean fewer missed appointments, more timely interventions and better continuity of care.
Houston Style Magazine applauds all six partners for recognizing an essential truth: healthier communities are built not only through outstanding institutions, but through outstanding connections.
Because in Houston, healthcare should never depend on who knows whom. It should depend on everyone working together.
